We can reimburse you for flu vaccinations received anywhere. Just pay for your own vaccination, keep the GST receipt, and complete the Vitality Works reimbursement form.

Print the reimbursement form now.

When the reimbursement form opens, press the Ctrl key on your keyboard, and the P key simultaneously. This will open the print window. Ignore the message about low quality printing and click 'Print'.

Reimbursement process

Once you have completed the Vitality Works reimbursement form, you will need to attach the GST receipt and send it back to Vitality Works.

Please scan and email the form and receipt to: fluvaccinations@vitalityworks.co.nz 

Your reimbursement will be made within 20 working days given you have provided all the required information.

Please note

• If you choose the reimbursement option, you must pay for your flu vaccination when you have it done.

• You need to remain under observation for up to 20 minutes after your vaccination.

• Please wear appropriate clothing - the pharmacist needs to access your upper arm to administer the vaccine.
